Medical Forms
Please download, print, review, and work with your medical providers to fill out the following forms as necessary. All forms are also available on the District 36 registration website. Return the completed forms to your school nurse by the first day of school, or within 30 days if you are registering mid-year.

Health Information includes immunization guidelines, grade level health form and immunization needs, area resources, medical and religious exemption information
Health Exam Form required for Junior and Senior Kindergarten students, 6th grade students, and all new students to the district
Dental Exam Form required for Senior Kindergarten, 2nd and 6th grade students, and all new students to the district
Eye Exam Form required for Senior Kindergarten students and all new students entering a State of Illinois school for the first time
Medication Permit required for any student needing prescription or over the counter medication during the school day
