All Kindergarten and sixth grade students MUST have a new Certificate of Child Health Examination Form on file with your student’s school health office prior to the first day of school. The exam must be dated after August 29, 2010. All Kindergarten and sixth grade students... MUST have a new Certificate of Child Health Examination Form on file with your student’s school health office prior to the first day of school which is Monday August 29, 2011 (this exam must be dated after 8/29/10).
